More specifically, it's the fact that through his constant inaction, he indirectly contributes to the deaths of everyone he knows and loves time and time again. The end result is a whole lot of helplessness, some grade-a depression, and a hefty dose of bottled-up guilt. Oh right, and he's also incredibly emotionally dependent on his brother Papyrus, who is at Sans's current canonpoint dead and also not in the city with him. Which means his monster is gonna look...well, it'll look something like this (warning for body horror in the image). He'll mostly wanna keep moving between the various temples because Feeling Things is Hard and He Doesn't Want It. Anyone want a weird teleportation ride to a temple? Provided you don't make him too suspicious, Sans might be willing to oblige.
