Most predominately, he's afraid of something happening to his little brother, so he'll be hallucinating his brother's dead body a lot. If anyone wants to have a fear-combining thread with your character's fear having killed his brother, I'd be 100% game! He may also have a moment or two of fear for his older brother, though those threads will be more fear and confusion than they will be outright screaming-upset fear.

As far as other fears go, he's not quite as afraid of himself as he is afraid that others will discover what goes on in his head. To sum up: lots of PTSD and bird monster personifications of self loathing?
